Long-Term Care
Bronx Park's staff is dedicated to the delivery of long term care with dignity. The facility offers long-term residents spacious dining rooms, lounges, and a large activity center where they develop friendships with both staff and peers.
AdmissionsNew admissions are welcomed by staff and are given an opportunity to learn about services and life at Bronx Park.
Clinical CareAn interdisciplinary team which includes medical, nursing, social services, rehabilitation, and dietary staff individualize a plan of care for each resident. Nutritious meals are served with attention to individual preferences. Each meal, we provide a number of options to ensure that each resident's preferences are honored. Individual and family counseling are provided by the Social Services department.
RecreationThe Recreation department assists each resident in developing a schedule of activities that reflects their own interests and hobbies. Bronx Park Rehabilitation and Nursing Center provides activities seven days a week. Evening activities are also offered. Sporting trips, holiday celebrations, and community trips are but some of the special highlights. Recreational programs are tailored to meet each individual's needs. Activities are designed to both enhance quality-of-life and to promote independence and self-expression. Individual recreation, group activities, and community trips maintain a link with the resident's lifestyle. Religious services are provided for a variety of faiths. Residents participate in diet selections through Food Committee meetings, and are encouraged to attend our Resident Council meetings to discuss the events of the facility and speak to the staff.
Family InvolvementBronx Park's team encourages families to participate in the resident's life as much as possible. Loved ones may visit 24 hours a day, providing flexibility for family. Families are invited to attend the interdisciplinary care planning meetings. Families are also welcome to attend parties or on holidays so that they may celebrate with their loved ones.
